Drop-box will be located at the Bellevue Sherman Clay. Forms will be collected at 5 p.m. on Monday, January 7, 2008 . Forms may be mailed to Elena Romanoff, postmarked no later than Jan 3rd, 2008. Email Elena Romanoff for her address.

WSMTA form with student names, levels, and fees
Chapter worksheet
One check for all student fees made out to Eastside Chapter WSMTA
